2016 13th Annual Dudley Conner Golf Scramble

The Kentucky Public Health Association is hosting the 13th Annual Dudley Conner Golf Scramble on Monday, June 27, 2016.  It promises to be a fun and rewarding event, so mark your calendars today!  Proceeds will benefit the KPHA scholarship funds.

Tee Time:  Shotgun Start 1:00 pm (EDT)

Lunch:  11:30 am (EDT)

Cost:  $75 per person; includes green fee, cart, lunch, refreshments and prizes

Awards:  Top Two Places, Gross and Net

Prizes:  Hole in One (Grand Prize), Longest Drive, Longest Putt, Closest to the Pin, Door Prizes, Casino Hole Contest

Mulligans and Skirts:  $5 each sold at the scramble.  Limit two per player (Cannot be used on the "hole-in-one contest)

Registration:  Team slots are limited.  Registration must be received by Monday, June 13, 2016.  Complete the registration form and submit payment on-line or send check to:  KPHA Golf Scramble, P.O. Box 4647, Frankfort, KY 40604-4647.
